Resource Referral Form

This form is for case managers to request referrals to services required by clients in CoC-funded housing programs who are either looking for housing or need help maintaining it. CAFTH staff will not directly refer clients or guarantee housing referrals. Our aim is to assist case managers in identifying the appropriate services to support their clients’ housing stability.

The Affordable Low Barrier Housing Program is a comprehensive housing initiative designed to address the diverse needs of our community. Due to low vacancy rates and high rental costs, households with rental vouchers often need help locating a landlord who will accept the subsidy. Through this service, the Community Alliance for the Homeless (CAFTH) Affordable Low Barrier Housing (ALBH) Program was created to incentivize landlords to rent their units to these individuals and families.
