Small Group Feedback Sessions

Community Alliance for the Homeless (CAFTH) staff hosted multiple small group sessions to gain project type specific feedback on the monitoring tool used for our community.

Monitoring Workshop Series

CAFTH staff hosted five workshops throughout the month of April to provide insights in what to expect from each part of the monitoring process.

Agencies Receive & Return Documents

CoC agencies received the monitoring packet which includes a cover sheet with instructions, the tool, an agency self assessment, intent to renew form, and document checklist. They have two weeks to upload files to their folders.

Monitoring & Client Home Visits

CAFTH staff will visit the agencies during their agreed upon time to review all documents and HMIS files. There will also be an unscored interview to review 3 year project trends and the agency’s self assessment and a visit the home of a client in each program who volunteers to participate.

5.8.25Monitoring office hours will be held virtually via Zoom on May 8th, 2025 from 10:00am-11:00 am then 11:00am-12:00 pm. These sessions are optional and informal (no presentation will be given) and are solely to provide an open forum for asking questions.
