After calling one of the 5 system navigators shown below, you will be given a brief assessment to determine eligibility for the navigation program. If you are eligible, you will be assigned a navigator who will contact you as soon as possible (ideally within 1-2 days). The purpose of the Youth System Navigators is to assist you in obtaining secure and stable housing. The Navigator will connect with you in whatever way is more accessible to you – whether that is via text, phone call, email, or an in-person visit in a safe location of your choosing. The Navigator will help you identify resource options and build a housing plan. If necessary, the Navigator will also administer the VI-TAY assessment to determine eligibility for specific housing options. Your navigator will be available to provide support and connection to resources until you have secured safe and stable housing.

De’Miracle Tabb is a compassionate social work professional currently employed at Communities In Schools, where she brings several years of case management experience to the individuals and families she supports. She graduated from the University of Memphis with a degree in Social Work, which laid the foundation for her commitment to service and advocacy. She has a passion for helping others navigate difficult seasons of life with hope and confidence. Using a strengths-based approach, De’Miracle focuses on identifying and building upon the unique abilities and resilience within each person she serves. For me, this work is personal because it’s about building real connections, showing up consistently, and creating a sense of belonging for youth who often feel overlooked. Joshua Johnson is employed by the SchoolSeed Foundation and is a native Memphian. Since graduating from Central High School and Morehouse College, Joshua has been dedicated to giving back to his community and working with youth and underserved individuals and families through his roles with the Memphis Ambassadors Program as a Youth Specialist and Memphis-Shelby County Schools as a Transition Specialist. When he’s not working on making Memphis a better place for all, he focuses on his creative endeavors as a musician, model, and clothing designer.

Stephanie has a Bachelor’s degree in Human Services and a Master’s in Psychology with an emphasis in Life Coaching. In 2014 she started working for Catholic Charities in Las Vegas, NV working in the men’s homeless shelter. She helped homeless men and homeless Veterans get into housing, find family members, start school, and get work experience through the Catholic Charities work program. Then she moved to Help of Southern Nevada as Case Manager for housing.  She was able to house homeless men and women. She moved back to Memphis, TN, in 2018 to be closer to family. Now she works as a Youth System Navigator for the YHDP through Le Bonheur Children’s Hospital.

I am uniquely trained and specialized to engage in a broad range of activities including Mental Health First Aid, First Aid, Motivational Interviewing, CPRP, Youth and Families services, and ADA. I currently work at Catholic Charities of West Tennessee. My focus is on helping every individual dealing with disabilities and/or social issues get the services they need and/or want.
